Referral & Admissions Process
Our policy is to consider young people of either gender
who are aged between 10yrs and under 19 yrs who would,
in our opinion benefit from the placement.
Unfortunately, we are unable to care for young people
with severe physical disabilities or a diagnosed mental
illness that if untreated may place the young person
and others at risk,
We will require all relevant background information
to enable us to make an informed decision as to whether
we are able to offer the appropriate support.
New Reflexions offers a unique level of commitment
to our young people and we believe that if we make an
informed decision to work with a particular young person,
we have a responsibility to see that work through for
as long as it is deemed by all involved to be in their
best interests.
An initial telephone call or email to New Reflexions
Head Office will determine whether there are any vacancies
within our organisation. If the initial conversations
indicate that the young person meets our admission criteria,
the placing authority should return a completed Referral
form together with appropriate supporting documentation.
Supporting documentation should include:
- Referring Social worker's assessment report (initial
and or core assessment, Pathway Plan)
- D.O.B
- Legal Status
- Social History/chronology
- Psychological / Psychiatric reports
- Court Proceedings, (past and outstanding) also
previous convictions list
- Medical History
- Current Risk Assessment
- Education Report
